Health insurance contains a lot of information. The harder problem is understanding what that information means for someone actually managing a chronic condition.

BenZen Research examines how ACA Marketplace health plans work for people managing diabetes.

We ask practical questions about plan design, prescription drug coverage, cost sharing, formularies, and the information consumers encounter while comparing health insurance. Then we use public data and plan documents to look for the answers.

Our work is independent and non-peer-reviewed. We publish our methods, sources, and limitations so the findings can be evaluated on their own terms.

What the SBC Diabetes Example Can Tell Us

An exploratory comparison of 2026 Marketplace health plans in Dallas County, Texas

When people shop for health insurance, they commonly compare premiums, deductibles, metal levels, and out-of-pocket limits. Some plans even put “diabetes” directly in their names.

But do those signals identify the plans that look most favorable when the same standardized diabetes-care scenario is applied?

We examined nine 2026 ACA Marketplace plans in Dallas County and added the standardized “Managing Joe’s Type 2 Diabetes” coverage example found in each plan’s Summary of Benefits and Coverage.

Key finding

In this exploratory sample, none of the four diabetes-branded plans had the lowest patient responsibility in the standardized Type 2 diabetes example.

The analysis also found substantial differences among plans with the same metal level and showed why a plan’s headline medical deductible does not necessarily explain how routine diabetes care will be treated.

Our initial research focuses on the intersection of diabetes and individual health insurance, including questions involving:

  • Marketplace plan design and cost sharing
  • Prescription drug and insulin coverage
  • GLP-1 coverage
  • Formularies and drug tiers
  • Prior authorization
  • Diabetes-specific plan benefits
  • Deductibles and out-of-pocket costs
  • The information consumers use to compare health plans

The goal is not to create another generic health-plan ranking.

We are interested in understanding how insurance benefits behave when viewed through the needs of people managing diabetes—and where existing insurance information may not make those differences easy to see.


Our Approach

BenZen works primarily with publicly available insurance information, including CMS Marketplace datasets, carrier Summary of Benefits and Coverage documents, formularies, plan documents, and other published benefit information.

We combine and examine those sources to answer specific, checkable questions.

Where appropriate, we publish the methodology, source material, assumptions, and limitations behind an analysis. Marketplace information can change, and related plan variants can differ. When we identify a material error in published research, we will correct it.
